Letter of Authorization
I, the undersigned customer of record (or an authorized representative thereof), hereby appoint WHITE OMAR LLC (operating as Daddy's Electric), its affiliates, agents, employees, subcontractors, and licensed supplier and consulting partners (collectively, the "Authorized Parties") as my exclusive energy consultant and designated broker of record for the account(s) and service address(es) listed above.
I authorize the Authorized Parties to request, receive, review, and use any and all account information from my current electric and/or natural gas utility and from any prospective or historical retail energy supplier, including but not limited to: account numbers, service addresses, meter numbers, rate class, tariff, load profile, interval and historical usage data (up to 36 months), demand data, tax status, current supply and delivery charges, contract expiration dates, payment history, and any other information necessary to prepare competitive supply quotes on my behalf.
I further authorize the Authorized Parties to communicate directly with my utility, the local distribution company, the ISO/RTO, and any retail suppliers or their affiliates on my behalf, and to use my information to solicit, negotiate, present, and (where I later provide separate written approval) execute retail supply agreements. This authorization does not, by itself, switch my supplier, change my service, or authorize any charges on my behalf — a separate written supply agreement signed by me is required before any enrollment.
This authorization supersedes any prior letter of authorization on file, is valid for twenty-four (24) months from the date signed, and may be revoked by me in writing at any time. I represent that I am authorized to act for the account(s) identified above and to grant this authorization.
By typing my name, drawing my signature below, and checking the box, I agree that my electronic signature is the legal equivalent of a handwritten signature under the federal E-SIGN Act and the Uniform Electronic Transactions Act (UETA), and I consent to receive records related to this authorization in electronic form.
